Managing Patient Comorbidities
Overview
This guide explains how to access a patient’s profile and add or manage comorbidities in the patient dashboard.
Purpose
The purpose of this guide is to help users correctly add, review, and manage patient comorbidities so that clinical conditions associated with the patient are accurately documented.
Executive Summary
Go to the Patients section from the sidebar.
Open View Patients and select a patient from the overview.
Access the Patient Dashboard.
Navigate to the Comorbidities tab.
Use Add Comorbidity to search and select conditions.
Add optional clinical notes if needed.
Review the Comorbidities History and manage visibility of removed entries.
Detailed Step-by-Step Guide
Step 1: Open the Patients Section
Go to the Sidebar and locate the Patients tab.
Click View Patients.
The Patients overview allows you to quickly locate and access patient records.
Step 2: Select a Patient
From the Patient Overview, select the patient you want to update.
This will open the Patient Dashboard.
From the dashboard, you will see the following tabs:
Profile
Appointments
Schedule Appointment
Orders
Comorbidities
The dashboard provides a centralized view of the patient’s information and clinical activity.
Step 3: Open the Comorbidities Tab
Click the Comorbidities tab on the patient dashboard.
From this section, you can add and manage patient comorbidities.
Step 4: Add a Comorbidity
Click the Add Comorbidity button.
A search bar will appear where you can search for the comorbidities to add.
Start typing the name of a condition, such as:
Diabetes
Vascular disease
Age-related conditions
As you type, a checklist-style dropdown will appear. From this dropdown list, you can select one or multiple comorbidities that apply to the patient.
You can also search using broader condition categories. Typing terms such as diabetes, cardiovascular, or obesity will filter the available comorbidities within those classifications.
Once the dropdown appears, check all relevant comorbidities. Selected items will be added to the patient’s assessment.
Step 5: Add Clinical Notes (Optional)
If necessary, enter clinical notes related to the comorbidity.
Clinical notes provide additional context that may help support documentation or care coordination.
Step 6: Review Comorbidities History
Within the Comorbidities tab, you will also see the Comorbidities History.
This section displays previously recorded comorbidities.
You can show or hide removed comorbidities by using the toggle switch.
Viewing removed comorbidities can help maintain an accurate record of past patient assessments.
