1. Prepare the Demographic/Face Sheet
Ensure that your document includes the following information:
A. Patient Information
Name
Address
Phone Number
Date of Birth
Gender
B. Insurance Information
Primary Insurance
Secondary Insurance (if applicable)
Tertiary Insurance (if applicable)
C. Procedure Information
Procedure Name
Anatomical Side
ICD 10 Code
Incision Length (if the procedure is a one-off case)
Provider Name
Date of Service- Incision Guide: Submit the incision guide as it is mandatory before any further steps can be taken.
2. Submission Methods
You can submit the completed face sheet by any of the following methods:
A. Upload to the Portal
Log In: Access our secure portal using your credentials, and click the patients page.
Navigate: Go to the "Patient Demographics Upload" section.
Upload Document: Attach your completed face sheet and submit.
Confirmation: Look for the confirmation message to see if succesful
B. Email
Attach Document: Email your completed face sheet as an attachment.
Send: Use the designated email address provided by your coordinator.
Confirmation: You will receive an email confirmation once your submission is processed.
C. Fax
Prepare Fax: Ensure your document is legible and includes all required fields.
Fax Number: Send your fax to 937.347.5042.
Confirmation: A confirmation call or email will be sent to confirm receipt of your fax.
3. After Submission
MedFlow IO Scheduling and Approval Process
Once the patient demographics and incision guide are received, the MedFlow IO team manages scheduling internally. Patients are then added to the schedule, and their eligibility status becomes visible. Regularly monitor this schedule to stay updated on patient approvals and their subsequent steps.
No Additional Steps Required: Once the demographic/face sheet is submitted using any of the methods above, no further action is required on your part.
Processing: The MedflowIO team will take over from here. They will:
Create the procedures.
Input the patient information.
Set up the appointment.- Once the patient demographics and incision guide are received, the MedFlow IO team handles the scheduling internally within the system.
Following this, patients are added to the schedule, making their eligibility status visible to you.
Ensure you monitor the schedule to track patient approvals and their next steps in the process.
Note: There is no need for manual data entry by the providing groups once the face sheet has been submitted.- Important Warning: Do not set any dates or appointments in the system until a surgery is linked. Doing so may cause patients to disappear from the system.
